In Class II malocclusions with severe mandibular deficiencies and minimal or no lower incisor crowding or compensation, the ROD-2 appliance is used, which allows distraction posterior to the molars. The ROD-2 device is a hybrid appliance consisting of an anteriorly activated distraction frame connected by removable attachments anteriorly to a crown on the 1st molar and posteriorly to a mini plate secured by screws to the ascending ramus. Additional anchorage may be obtained with struts extending from the crown anteriorly to the 2nd bicuspid and posteriorly to the 2nd molar.
