The Modular Internal Distraction System or MID System, developed in conjunction with Cohen, permits distraction and manipulation of the entire craniofacial skeleton, including the midface, cranium, and mandible. This allows the surgeon extreme flexibility in osteotomy design and intraoperative planning (Cohen et al., 1995, 1997b). Similar to the DynaForm, this system consists of a universal distractor frame (15 mm and 30 mm long) with a flexible activation cable, which can be brought through hidden percutaneous sites in the scalp, pre- or postauricular, or intraoral regions. The distraction frame is easily assembled to 1.7 mm Mini Wurzburg 3-D Mesh plates by 1.6 mm connecting screws for intraoperative adaptation of the appliance based on the clinical application and/or anatomical location.
